As part of its 20th anniversary highlights, PhilHealth launched its Member Inquiry facility on its homepage http://www.philhealth.gov.ph/ last 2015.
Access your contribution history or print a copy of your MDR in one click. |
How it Works
To enjoy the convenience of PhilHealth’s online portal, members must first register through https://memberinquiry.philhealth.gov.ph/info/validate.php. Make sure to note your PhilHealth ID number before creating an account and provide a valid email address and mobile number.
After registration, an email will be sent to you providing you with a system generated PIN number and a password.
VERY IMPORTANT: Your member portal PIN is different from your PhilHealth ID number. If possible, do not delete the system email from PhilHealth containing your PIN and password or keep a record of it somewhere as there is no convenient way of recovering it at the moment.
You may then login through the homepage, the member inquiry login form is found on the right sidebar, or bookmark this URL http://www.philhealth.gov.ph/services/inquiry/. Once logged in, you can change your password, view your contributions history, and print / download a copy of your latest MDR.
For those who need to update their personal information (change of last name for those recently married, change of address, membership type, or employer information), you still need to go through the process at your nearest PhilHealth office or with the assistance of your company HR.
Not yet a PhilHealth member? You can also enroll online. Visit the Online Services section for more information.
